You DO need to have current updated immunization information prior to school entry. Immunization information should be a copy from your physician's records, not a baby book. If you are not sure what immunizations your child needs, please contact your physician/health care provider.

School Physicals - If your child is new to the Chenango Forks District, or entering either Pre-K or kindergarten or entering second grade, and you would like the school physician to complete a health appraisal of your child, please contact me either by phone or by web contact and I would be happy to arrange that for you.

Physical Forms - The form is on this site and can be found under forms, then printed at home. If you would like one sent home with your student, please send an email, note or call me at 648-7531 and I would be happy to do that.

According to NYS Education Law §903, public schools are to ask for a dental health certificate of students, at the time of school entry and in grades K, 2, 4, 7, and 10, declaring their dental health condition; beginning in school year 2008-2009.

If your child currently receives dental care the certificate should be completed by their dentist.
