Student should get the application and physical fitness form from the high school (visit the nurse's office during the school year, or the main office over the summer. Forms can also be downloaded and printed here:

Student should bring the form home for the parent/guardian to fill out Part 1.
If the student has a physical filed with the school within the past year from the application date, that one case be used. Otherwise, the student will need to have a physical done by their health care provider or the school physician. The school physician visits once a week during the school year.
Working papers are in two categories: 14 and 15 years of age, and 16 and 17 years of age. The student needs to fill out applications for each category at the appropriate time.
Once the application is signed by the parent and the physical is done, return the application to the school and it will be processed. The student will be given an age-appropriate card required by employers. The student's signature will be needed to complete the process.
